Q: If the movie was based on a manga, what was the manga based upon?

The manga was based upon an actual cat Baron statue that Hiiragi Aoi owns. The doll was sitting in a store, and Hiiragi very much wanted to buy it. On the day she did decide to buy it, however, it had already been sold. She later found out that the buyer of the doll was her fiance (now her husband), who had bought it for her.

Q: Is the movie Neko no Ongaeshi a sequel to this movie?

No.
Many people believed that the movie Neko no Ongaeshi (known as The Cat Returns in English) is a sequel to Mimi wo Sumaseba due to the Baron being in the movie. In fact, Neko no Ongaeshi is simply another adaptation from yet another of Hiiragi Aoi's manga of the same title.
As a consolation though, Hiiragi Aoi was reported to have said that Neko no Ongaeshi was one of the many stories that Shizuku has written. So in sense, it may very well have been a sequel.

Q: Are all the locations in the movie real?
|
|
The basis for most of the locations is Tama city, with a lot of references taken from the area around Seiseki-Sakuragaoka station. However, not all the locations exist in real life. For example, the actual municipal library is nowhere near where it was in the library, while the Atelier does not exist at all. The same goes for Shizuku's fictitious home town, Mukaibara, which was copied from the manga.

Q: What about her school?
|
|
The credits include a reference to a certain "Koganei Shiritsu Koganei Daiichi Chuugakkou," or #1 Koganei Municipal Middle School. It is possible that this middle school was used as a model for her school.
